Dumova Tennis LLC — Waiver & Release

I confirm that I (or my child) am physically able to participate. If any condition appears unsafe, I will stop participation immediately and notify staff.

I understand tennis and athletic training involve inherent risks, including serious injury (e.g., falls, collisions, being struck by balls/racquets, overexertion), permanent disability, or death. I voluntarily assume all risks, known and unknown.

To the fullest extent permitted by law, I release and waive any claims against Dumova Tennis LLC and its owners, coaches, staff, contractors, agents, and volunteers (the “Released Parties”) for any injury, illness, loss, theft, or damage arising from participation in the Activities, including claims based on negligence.

If I (or anyone on my behalf) makes a claim against any Released Party, I agree to indemnify and hold them harmless from related losses, damages, and reasonable attorneys’ fees to the fullest extent permitted by law.

I understand Dumova Tennis LLC does not provide medical insurance. In an emergency, I authorize Dumova Tennis LLC to obtain medical care for me (or my child), and I am responsible for all related costs.

If any portion of this waiver is found unenforceable, the remainder remains in effect.
